Keep your own words within reach

Write down what you want the clinician to understand. What brought the concern to your attention, and what do you want explained? Use the words you’d use in conversation, and mark anything you’re unsure about.
Have your current medicine information, allergies and questions nearby. Describe the routine you actually follow. The aim is to give the clinician a clear picture, rather than change anything for the visit.
HHS also suggests gathering medical history and pharmacy contact information. A clinician you haven’t seen before may be missing some of your history. Ask which records they need and how to share them through the office.
